CONSENT TO PARTICIPATE
As a condition of participation in intercollegiate athletics at Ottawa University, each student-athlete will be required to sign a consent form agreeing to undergo drug testing and authorizing release of test results in accordance with this policy (See Appendix A). Failure or refusal to sign this consent or comply with the requirements of this policy may result in the student-athlete’s athletic scholarship being revoked and the termination of eligibility to participate in intercollegiate athletics at Ottawa University. Student-athletes will be given the opportunity to ask any questions regarding the information contained in the policy, the testing program, or other related issues prior to signing the Ottawa University drug-testing consent form. The NAIA Policy is separate and distinct from the OU Policy and a signed consent form is required of all student-athletes.
